ABSTRACT Objective: Evaluating the clinical results of bioactive glass S53P4 putty for the treatment of cavitary chronic osteomyelitis. Methods: Retrospective observational study, including patients of any age with clinical and radiological diagnosis of chronic osteomyelitis, who underwent surgical debridement and implantation of bioactive glass S53P4 putty (BonAlive® Putty, Turku, Finland). Patients who underwent any plastic surgery on the soft tissues of the affected site or had segmental bone lesions or septic arthritis were excluded. Statistical analysis was performed using Excel®. Demographic data, as well as data on the lesion, treatment, and follow-up, were collected. Outcomes were classified as “disease-free survival,” “failure,” or “indefinite.” Results: This study included 31 patients, of which 71% were men and had with a mean age of 53.6 years (SD ± 24.2). In total, 84% were followed-up for at least 12 months and 67.7% had comorbidities. We prescribed combination antibiotic therapy for 64.5% of patients. In 47.1%, Staphylococcus aureus was isolated. Finally, we classified 90.3% of cases as “disease-free survival” and 9.7% as “indefinite.” Conclusion: Bioactive glass S53P4 putty is safe and effective to treat cavitary chronic osteomyelitis, including infections by resistant pathogens, such as methicillin-resistant S. aureus. RESUMO Objetivo: Avaliar a atividade do vidro bioativo S53P4 em pasta no tratamento de osteomielite crônica. Métodos: Estudo observacional retrospectivo, com inclusão de indivíduos de qualquer idade com diagnóstico clínico e radiológico de osteomielite que realizaram tratamento cirúrgico com limpeza e desbridamento, seguido do preenchimento da cavidade com biovidro S53P4 em pasta (BonAlive ® Putty, Turku, Finland). Foram excluídos pacientes submetidos a procedimentos de cirurgia plástica nos tecidos moles do local afetado, com lesões ósseas segmentares e com presença de artrite séptica. A análise estatística foi realizada em Excel ® . Foram coletados dados demográficos, sobre a lesão, o tratamento e o acompanhamento. O desfecho foi classificado em “sobrevida livre de doença”, “falha” ou “indeterminado”. Resultados: Dos 31 pacientes analisados, 71% eram homens, com idade média de 53,6 anos (DP ± 24,26). Do total, 84% foram acompanhados por no mínimo 12 meses, e 67,7% apresentaram comorbidades. A terapia antibiótica combinada foi realizada em 64,5% dos pacientes, sendo o patógeno mais frequente o Staphylococcus aureus (47,1%). Ao final, 90,3% dos pacientes obtiveram “sobrevida livre de doenças” e 9,7% foram considerados “indeterminados”. Conclusão: O vidro bioativo S53P4 em pasta é seguro e eficaz no tratamento da osteomielite cavitária e de infecções por patógenos resistentes, incluindo o S. aureus multirresistente. CONTEXTO E OBJETIVO: A incidência de infecção superficial no trajeto dos fios e pinos junto à pele é uma complicação freqüente em pacientes em tratamento pelo método de Ilizarov. Este estudo objetivou avaliar a eficácia da aplicação diária da solução tópica de polivinilpirrolidona-iodo a 10% na interferência de infecção nos orifícios dos fios de Kirschner e pinos de Schanz de pacientes em uso de fixador externo de Ilizarov comparada à limpeza desses orifícios somente com soro fisiológico a 0,9%. TIPO DE ESTUDO E LOCAL: Ensaio clínico randomizado controlado, no Ambulatório de Ortopedia e Traumatologia do Hospital São Paulo e Centro de Ortopedia e Traumatologia de Jundiaí. MÉTODOS: 30 pacientes foram tratados pelo método de Ilizarov: 15 orientados a realizarem o curativo dos orifícios dos fios e pinos com soro fisiológico a 0,9% e 15 com soro fisiológico a 0,9% mais polivinilpirrolidona-iodo tópico a 10%. Os pacientes foram avaliados nos retornos ambulatoriais, para identificação dos sinais e sintomas de infecção superficial nos orifícios dos fios e pinos. Amostras foram obtidas para cultura e antibiograma na presença de exsudato com pus à expressão. RESULTADOS: Aplicado os testes exato de Fisher e qui-quadrado não foi possível encontrar associação estatisticamente significante da intervenção da solução tópica de polivinilpirrolidona-iodo na prevenção de infecções nos trajetos dos fios e pinos. CONCLUSÕES: A solução de polivinilpirrolidona-iodo tópica a 10% quando aplicada diariamente no orifício dos fios de Kirschner e pinos de Schanz à pele não reduz a incidência de infecção superficial nesses trajetos quando comparada com pacientes que realizam a remoção mecânica de sujidades com soro fisiológico a 0,9%.

CONTEXT AND OBJECTIVE: Superficial infection at wire and pin insertions in the skin is a frequent disorder among patients utilizing the Ilizarov method. The objective of this study was to evaluate the effectiveness of daily topical application of 10% polyvinylpyrrolidone-iodine solution against infections of the holes for Kirschner wires and Schanz pins among patients using Ilizarov external fixators, in comparison with cleaning these holes only with 0.9% sterile physiological saline solution. DESIGN AND SETTING: Controlled randomized clinical trial, in the Orthopedics and Traumatology Outpatient Clinic, Hospital São Paulo, and Orthopedics and Traumatology Center of Jundiaí. METHODS: 30 patients were treated using the Ilizarov technique: 15 were instructed to apply 0.9% physiological saline dressing on the wire and pin insertions and 15 to apply 0.9% physiological saline plus 10% polyvinylpyrrolidone-iodine. Patients were evaluated at outpatient return visits for identification of signs and symptoms of superficial infection at wire and pin insertion sites. Samples were collected from cases of purulent exudate secretion, for culturing and clinical tests. RESULTS: The chi-squared and Fischer exact tests were applied, but no statistically significant association between the intervention of topical polyvinylpyrrolidone-iodine solution and the prevention of infections at wire and pin insertions could be found. CONCLUSIONS: Topical 10% polyvinylpyrrolidone-iodine solution applied daily to Kirschner wire and Schanz pin insertions did not reduce the incidence of superficial infection at these holes, in comparison with mechanical removal of dirt using 0.9% physiological saline solution.

Foram tratados 28 pacientes num total de 29 pés, com idade média de 25,8, predomínio do sexo masculino 16 pacientes (57,2%), cor branca 25 pacientes (89,2%), lado direito 16 pacientes (57,2%). Quanto à etiologia houve um predomínio das seqüelas de fraturas de calcâneo 11 pacientes (37,9%) e poliomielite 11 pacientes (37,9%), as outras causas foram necrose do talus 2 pacientes (6,9%), barra óssea 2 pacientes (6,9%), artrose primária 2 pacientes (6,9%) e paralisia cerabral 1 paciente (3,5%). O tempo médio de consolidação foi de 3,5 meses. Foram utilizados enxerto ósseo em 11 pacientes (44%). Houve necessidade em 2 casos (6,9%) de se promover o alongamento do calcâneo para se aumentar o pitch do mesmo. A montagem é simples e se baseia em 2 anéis com fixação transóssea através de 2 fios olivados no talus e 2 no calcâneo. Como resultado foi obtido 100% de consolidação, ausência de dor e osteoporose e boa sensibilidade do pé. Das complicações encontradas além das costumeiras ou próprias do método 1 (3,5%) paciente apresentou infecção profunda em 1 dos fios. O método mostrou-se eficiente independente da etiologia com 100% de consolidação além de permitirem uma certa independência ao paciente durante o tratamento.

There had been heated 28 patients in a total of 29 feet, with na age average of 25,8, predominance in the quality of being male 16 patients (57,2%), white color 25 patients (89,2%), right side 16 patients (57,2%). As the ethiology there were a predominancy of the calcaneum fracture´s consequence 11 patients (37,9%) and poliomyelitis 11 patients (6,9%), bone bar 2 patients (6,9%), primary arthrosis 02 patients (6,9%) abd cerebral palsy 1 patient (3,5%). The average time of consolidation was 3,5 months. There had been used bone graft in 11 patients (44%). There were the necessity to promote the calcaneum extension to raise the pitch of itself, in 2 cases (6,9%). The mounting is simple and it relies on 2 rings transbone settlement through 2 strings fixed in the talus and 2 in the calcaneum. As the result was obtained in 100% of consolidation, pain´s absence and osteopethosis and feet´s great sensibility. In the complications there were found, besides the usual or method itself 1 (3,5%) patient. Showed deep infection in 1 of the strings. The method has showed efficient, independent of the ethiology with 100% of consolidation beyond to permit a certain independancy of the patient during the treatment.
